The completion of the I-693 form is mandatory for individuals applying for a Visa, Green Card, United States Citizenship, or any adjustment in status.

Typically, a blood draw and urine test are required, with specific tests tailored to the patient’s age and personal medical history. After the initial visit, the applicant will return to the clinic for a second follow-up visit. The entire process may span up to 14 days.

Instructions for Your Physical:
Ensure you bring all medical records, including vaccination records, as well as your Passport/VISA. It is essential to have these documents translated into English or bring a translator along to assist in the process.
